When and how do you get your health insurance card?
If you enrol in public health insurance, we will issue you an insurance card after the Health Care Surveillance Authority reviews your application form.
If you change health insurance company (which you can do once a year by September 30), we will send you a new insurance card at the beginning of December. You will officially become a Union insured person on January 1 of the following year, and then you can start using your insurance card.
In both cases, we will send your insurance card by registered post to the address listed on your application form or to the branch you specified in your application. If you are a foreigner with a residence permit and you change your health insurance company, your address will be automatically updated based on the data in the Register of individuals maintained by the Ministry of the Interior of the Slovak Republic. Therefore, you do not need to report your address change.
In other cases, you can report a change of delivery address only before the insurance card is in production. You can do this by emailing your details to preukazy.zmenazp@union.sk.
Digital Health Insurance Card
You will also have access to a digital health insurance card, which you can find in the mobile application.
Mobile Application
With our mobile app, you can easily and quickly access all essential information, especially regarding your health and health insurance. We will send you the login details for the mobile app along with your insurance card. The mobile app always keeps handy:
- All discounts and benefits, with the ability to generate discount codes directly in the app.
- Your insurance card, prescriptions for pickup, and a list of preventive check-ups you are entitled to.
- An overview of insurance payments, statements of outstanding amounts, and the option to pay your insurance dues.
- Zdravomer feature, which provides basic information about your health.

Always carry your insurance card with you, as you will need to present it at every doctor visit and when picking up prescription medications. The health insurance card allows you to access necessary health care within the scope of valid legislation at facilities that are part of Slovakia’s public health insurance system. We recommend that our insureds prefer healthcare providers who have a contract with us. Standard healthcare guaranteed by the state through compulsory monthly insurance contributions is covered. If you visit a non-contracted facility, you will pay for standard healthcare services because the doctor does not have a contract with the health insurance company, so they are not entitled to reimbursement from public health insurance. Thus, you will end up paying twice, as you have already paid through your insurance contributions.
You can verify whether your doctor or specialist has a contract with our insurance in our mobile app under the „My Health – Doctors and healthcare providers“ section. This section is also accessible without signing in. Alternatively, you can contact our Customer Service Centre.
In case of loss or expiration of your insurance card, request a new one. You can do this through:
- your account in the mobile app,
- email at preukazy.zmenazp@union.sk (don’t forget to provide your personal details for correct identification and the exact address for card delivery or the branch where you will pick it up personally),
- in person at any branch,
- by phone at the customer service line 0850 003 333 (domestic calls) or +421 2 2081 1811 (international calls),
- by post, by sending a completed application to Karadžičova 10, 81453 Bratislava.
Your only responsibility is to report the change to your employer. If you have more than one employer, you are required to notify each of them. It is not necessary to cancel and return the previous health insurance card. There is no obligation to report the change of health insurance to your doctor or healthcare provider. Some providers appreciate being informed of the insurance change by the insured. If you decide to do so, it is enough to notify by phone or email.
If you are a foreigner without permanent residence in Slovakia, you are entitled to public health insurance only while you are employed in Slovakia. If you cease your employment activity, your public health insurance automatically expires. For instance, if you are insured with temporary residence and you terminate your employment on March 15 and find a new job starting April 1, you must submit a new application for public health insurance to insurance company.
